View, Download, or Print an Insurance Claim

Therapyzen's CMS 1500 feature makes it easy for you to create CMS 1500 claims and submit them (electronically or physically) to an insurance company for reimbursement for services. For providers in-network with Medicare or Medicaid, Therapyzen can be used to quickly create claims that can exported and submitted to CMS.

Download a Claim as a PDF 

Follow the steps below to export your claim as a PDF. 

1. Navigate to the Claims area of therapyzen.
2. Any claim that has passed therapyzen's initial scrub in the Claims Staging area can be printed to CMS.
   1. Click the Actions dropdown button and then click View / Print CMS 1500 
      
   2. The pdf preview will load with your CMS document.  From the preview you may print or download the pdf.
      

Printing a 'Blank' CMS form

Claims created in therapyzen will also match up with the standard CMS claim form. Please note, some insurers like Medicare & Medicaid do NOT accept photo copies of a form, or printed version of a form on regular paper. The form must be printed on a standard CMS 1500 pink claim form. This paper form is available from various retailers, such as Amazon, Staples, and Office Depot. 

To print a CMS on the formatted CMS 1500 pink claim form, click the Print Blank CMS Form button found in the CMS preview.
